This data provides a list of benefits allowable according to the definitions of the contract as provided by your insurance carrier. Your employer and insurance agent can also be helpful in answering questions about your available benefits. The insurance carrier or managed care company determines appropriateness of care based on your presenting problem(s). They have established guidelines for medical necessity and may certify a portion of your stay up front.

Rarely does a plan approve an entire treatment stay. Futures of Palm Beach will work with your insurance company to provide the requested clinical information, but Futures of Palm Beach has no control over the outcome of these reviews. Importantly, these reviews do not guarantee payment by your insurance carrier upon final billing.

Over the past few years, many states have required that insurance companies provide benefits for behavioral health. Futures has a high degree of experience working with insurance companies and we are able to use that experience to help fight for the highest reimbursement possible. It is very important to note that insurance polices vary drastically from one plan to the next and from one state to the next. For example, some insurance companies require:

  • That you must have completed outpatient treatment before considering residential treatment
  • That a co-pay must be made in addition to what they will cover. Co-pays are payments that your insurance company will ask you to pay before you come to residential treatment. Co-pay is different from a deductible, which is an out-of-pocket expense you may be asked to pay as part of the percentage that your insurance does not cover.
  • That you are only covered to stay in residential treatment for a certain amount of days per year or a certain number of days over your lifetime.
  • That you many only be covered for a limited dollar amount

What Does CARF Do? — CARF helps treatment centers achieve internationally recognized standards by working with them to improve the quality and value of their services.

The Accreditation Process — CARF conducts an on-site survey to evaluate the business and service standards of a facility. Then a facility must submit a Quality Improvement Plan (QIP) to address areas of improvement.
